 Saving time is important to me and there is nothing more annoying than to be waiting for a return phone call from me to schedule an appointment. Hopefully, you won't mind booking your own appointment, which often saves you time and allows multiple bookings.

Here are just a few things to know when making your appointment:

  •  Please arrive 10-15 minutes early to your first appointment to finish any remaining paperwork.
  • Life Tree Counseling Center accepts checks, cash, HSA and HRA account cards and all major credit cards.



"Life Tree does not fill out paperwork for FMLA, Short -Term Disability, Long - Term Disability,

Workman’s Compensation claims, or write letters for Emotional Support Animal

requests. These types of requests should be filled out specifically by your medical

doctor. This paperwork requires a medical diagnosis from a medical doctor."

  • 1st Appointment Options

    • 1st Appointment - EAP

      Please select this option if this is your FIRST EAP appointment. Bring the the name of the Employee Assistance Program, phone number and authorization code given to you by your EAP.

      This first visit to is designed to discuss your specific situation. Together, we will assess whether we are an appropriate fit and determine appropriate treatment goals.

      There is no copay/coinsurance for these sessions if we have the correct EAP information. Typically, your EAP will give you, 3-5 sessions to review and discuss work-life situations.

      Prior to your appointment, please download our Intake Packet and fill it out ahead of time. This will give us more time to talk about your needs during your scheduled session time. Once you schedule your appointment you will receive an email with the password needed to access the forms on the download page, so schedule your appointment then go check your Inbox!

    • 1st Appointment - Insurance $225

      Select this service if this is your first time visit utilizing your insurance benefits. 

      This first visit to is designed to discuss your specific situation. Together, we will assess whether we are an appropriate fit and determine appropriate treatment goals

      I will collect a copay and depending on your deductible and coinsurance. This varies and initially ranges anywhere between $45.00-$95.00. If there is a deductible you need to complete, I require $95.00 until we know how your insurance carrier applies the payment with provider discounts per billing code. 

      We take all major Credit card payments, HSA/HRA account cards, check and cash and payment is due at the time of  each session. 

      Prior to your appointment, please download our Intake Packet and fill it out ahead of time. This will give us more time to talk about your needs during your scheduled session time.

    • 1st Appointment - Self-Pay $175

      Select this option if this is your first visit with us and you will be paying out of pocket.

      This first visit to is designed to discuss your specific situation. Together, we will assess whether we are an appropriate fit and determine appropriate treatment goals.

      This is self pay option and payment is due at time of service. HSA cards can be used as payment. This appointment option employs a Self-Pay fee of $175 and your appointment will be 60 minutes in length.

      We take all major Credit card payments, HSA/HRA account cards, check and cash. Payment is due at the time of  each session and I will need to copy your driver's license at the first appointment. 

      Prior to your appointment, please download our Intake Packet and fill it out ahead of time. This will give us more time to talk about your needs during your scheduled session time.

  • 2nd+ Appointments (EAP or Insurance)

    • 2nd+Appointment(s) - EAP

      If you have already completed your FIRST EAP appointment, please select this service option if you want to continue to utilize your remaining, available EAP sessions. 

    • 2nd+ Appointment - Insurance $160

      If you have already completed your FIRST Insurance-paid appointment, please select this service option if you want to continue to utilize your insurance benefits. Copay will be collected and varies depending on your insurance carrier. 

      We take all major Credit card payments, HSA/HRA account cards, check and cash and payment is due at the time of  each session.

  • 2nd+ Appointments (Self-Pay)

    • 2nd+ Appointments - Self-Pay: 75 Minutes $175

      If you have already completed your first Self-Pay Appointment with us, please select one of these options for all subsequent self-pay sessions. Your appointment will be 75 minutes in length and this session fee is $175.

      We take all major Credit card payments, HSA/HRA account cards, check and cash and payment is due at the time of each session.

    • 2nd+ Appointments - Self-Pay: 60 Minutes $150

      If you have already completed your first Self-Pay Appointment with us, please select one of these options for all subsequent self-pay sessions. Your appointment will be 60 minutes in length and this session fee is $150.

      We take all major Credit card payments, HSA/HRA account cards, check and cash and payment is due at the time of each session.

    • 2nd+ Appointments - Self-Pay: 45 Minutes $120

      If you have already completed your first Self-Pay Appointment with us, please select one of these options for all subsequent self-pay sessions. Your appointment will be 45 minutes in length and this session fee is $120.

      We take all major Credit card payments, HSA/HRA account cards, check and cash and payment is due at the time of each session.

  • LTC Landing Pages

    • 1st Appt - EAP (My Frisco Counselor)
